Abstract Funding Acknowledgements Type of funding sources: None. Background Myocardial infarction (MI) with nonobstructive coronary arteries (MINOCA) is considered to be a puzzling clinical phenomenon unclear prognosis. This study aims compare the characteristics, impact arrythmias and prognosis MINOCA MI obstructive artery disease (MICAD). Methods Data was obtained from our university hospital database. Patients between 28 August 2018 31 December 2020 were included in study. Subjects divided 2 groups: 1 - MINOCA; -MICAD. Follow-up analysis death any cause, major adverse cardiac events (MACE: death, MI, stroke), readmissions due cardiovascular causes, in-hospital mortality. Median follow up 18 months. Results occurred 49 (11 %) out 443 pts MI. There no differences age (p=0.732). In group there more females (34.7% vs 22.8%, p<0.001), never smokers (30.6% 21.8%, lower rates hypertension (67.3% 73.8%, p=0.036), diabetes (18.3% 29.9%, hyperlipidaemia (28,5 % 35.7%, p=0.475), stroke anamnesis (2.0% 6.8%, preavious p<0.001). Symptoms at admission didn't differ two groups, however had mean heart rate (72±15 87±19, systolic arterial pressure (139±22 145±28, significant STEMI or NSTEMI: 55.1 44.9 53.8 46.2 MICAD (p=0.534). patients C-RP levels (19 56 mg/L, high-sensitivity troponin T levels: (112 376 ng/L, p<0.001) BNP (99±26.9 142±64.2, Left ventricular dysfunction (EF < 50 prevalent (16.3% 27.9%, need inotropic support (0% 4.8%, p=0.04). Atrial fibrillation (AF) often found (4.1% p <0.001). diffrenecs arrhythmias during admmision both groups. Death 2.8%, p=0.131) period (8.1% 10.9 %, p=0.369). difference MACE outcomes (p=0.612) readmission causes (p=0.310) Independent predictors for older (≥60 years), females, reduced LVEF atrial fibrillation. Conclusion prevalence classic risk factors, admission, T, preserved left ventricle EF, in- rate, but outcomes. Older age, are MINOCA. Further research needed provide evidence on optimal management.

متن کاملSudden arrhythmic death: from basic science to clinical practice
Sudden cardiac death refers to unexpected death attributable to a cardiac cause occurring within 1 h of the onset of symptoms (NICE, 2006). It often results from cardiac arrhythmias and is a major worldwide cause of morbidity and mortality. Arrhythmias account for 180,000 to 250,000 deaths per year in the United States and ∼70,000 deaths per year in the United Kingdom (NICE, 2006; Chugh et al.,...
